Sinampahan ng reklamo sa Office of the Ombudsman ng National Bureau of Investigation-Anti-Graft Division (NBI-AGD) ang isang opisyales ng Accreditation Committee ng PhilHealth Regional Office – National Capital Region (PRO-NCR).
Ang kasong isinampa ay paglabag sa Section 3(e) ng RA 3019 (Anti-Graft and Corrupt Practices Act). Inirekomenda rin ng NBI ang paghahain ng kasong administratibo sa mga respondent dahil sa paglabag sa RA 6713 o Code of Conduct and Ethical Standards for Public Officials and Employees.
Ayon kay NBI Officer-In-Charge (OIC) Eric B. Distor, Ang kasong kanilang isinampa ay bagay sa resulta ng kanilang ginawang imbestigasyon.

Nag- ugat ang kaso sa reklamo sa National Bureau of Investigation (NBI) ni Dr. Leilani B. Mercado-Asis na simulat noon kay Dennis Mas, Vice President ng PRO-NCR .
Ayon sa complainant, diumano’y may iregularidad sa renewal ng Health Care Provider accreditation.
Lumilitaw sa imbestigasyon na hindi nabigyan ng due process ang complainant.
Ang basehan kasi ng denial ng accreditation ay hindi muna dinidinig ng Arbitration Office ang kaso nito.
Wala rin umanong formal complaint o kaya ay pagkakaroon ng fact-finding investigation.
