· Fill-up the Application Form (can be downloaded at hhtps:// goo.gl/sluc7c, and the Self-assessment guide at www.tesda.gov.ph or you may ask our TESDA Provincial Office near your location);
· Attach 3 passport size pictures (colored, white background and write your name at the back); and
· Submit to the nearest TESDA Provincial Office or to Accredited Assessment Center.
